Bringing Nourishment, Connection, and Support to Watertown Families

As winter settles into the North Country and the holiday season begins, community partners across Jefferson County are joining forces to bring warmth, support, and resources to local families. On Saturday, November 22, 2025, ModifyHealth, Healthy Alliance, Urban Mission, and several community organizations will host the Harvest of Hope Turkey Giveaway — a drive-through event dedicated to supporting Medicaid members and connecting them to vital long-term services available at no cost.

A Community Effort Focused on Health and Hope

Harvest of Hope is more than a holiday giveaway — it’s an opportunity to raise awareness about the Social Care Network (SCN) and Health-Related Social Needs (HRSN) Program, part of New York’s 1115 Waiver Demonstration Amendment. These initiatives help eligible Medicaid members access essential supports that go far beyond clinical care.
