63M with hx of abdominal pain, bloating, gas, nausea, constipation and weight loss. Dx with IBS-C, GERD and to r/o gastroparesis. Self initiated low FODMAP diet on his own approximately one year ago with improvements in his symptoms from an IBS-SSS baseline of 320/500 to 180/500 at his initial appointment. He was primarily in need of support to identify which foods were triggers for him, so he could expand his variety and continue to manage/minimize his symptoms. He and his wife were planning a trip to Eastern Europe, and he was concerned with what to eat during his travels so he didn't trigger his symptoms.
After completing Elimination Phase, patient had additional decrease in his IBS-SSS to 100/500. He then implemented the Reintroduction Phase and identified a mild intolerance to mannitol and a moderate intolerance to fructans. I assisted him with tailoring (filtering) the Monash app for these intolerances to allow him to individualize the information provided. I also provided information on foods containing both of these FODMAP groups.
When asked what he would say to someone who was considering doing the 8-week MH Program, he said that he would "highly recommend it". "It was something that I needed" to help me to find out how to "narrow the foods that caused problems" for me. He and his wife shared that they have "learned a lot" and are now eating more fresh versus processed foods and are looking forward to their trip to Prague and feeling more confident about controlling his symptoms during their trip.
