Starting January 1, 2025, New York Medicaid members can get more support through the new Social Care Network (SCN) program. SCNs connect your health plan, doctors, and trusted community partners to help with everyday needs that impact your health - like food, housing, transportation, and more.

As the Community Based Organization in your region, ModifyHealth makes it simple:

• Connect you to the right services
• Provide support at no cost if you qualify 

It’s not replacing your Medicaid benefits - it’s expanding them, so you can live healthier and more independently.

If you are a New York State Medicaid Member, there is a new program focused on meeting your health-related social needs, called the Social Care Network (SCN) Program.

How it works...

Get Screened

We’ll ask you a few quick questions to better understand your needs and confirm eligibility. Topics may include housing and utilities, food security, transportation, employment, education and interpersonal safety.

Screenings are only done with your consent, and your information is used only to determine your needs and eligibility.

Find Out What You're Eligible For

If your screening shows unmet needs and you agree to move forward, a Social Care Navigator will complete an eligibility assessment.

They will reach out to you within 5 business days.

Get Connected

If you’re eligible and want services, your Navigator will connect you with the right provider who can help.

Services Are Provided

You’ll begin receiving the services you qualify for — these may be one-time, short-term, or longer-term (up to 6 months).

Follow Up

Your Social Care Navigator will follow up to make sure you received your services and that your needs were addressed.

What to know...

What are Health-Related Social Needs (HRSNs)?

HRSNs are things that can impact your health and well-being, such as:

• Lack of access to healthy food
• Unstable housing
• Financial stress
• Limited transportation

How does the SCN Program work?

Under this program, regional Social Care Networks (SCNs) bring together providers of all kinds to meet your HRSNs.

That means your primary care doctor, health plan, government agencies, and local social service providers (like food pantries or housing supports) work as one team to support your health.

What is ModifyHealth’s role?

ModifyHealth serves as a Community Based Organization in New York.

• We build and support the many SCNs across the state.
• We are also a service provider – screening you to understand your needs and connecting you to the right partners in the network.

What services are available through the program?

All services are provided at no cost to you, if you qualify.
Eligibility is determined through an HRSN screening and assessment.
Depending on the service, support may be short-term, one-time, or up to 6 months.

• Navigation Services
• Housing & Utilities Support
• Transportation Services
• Nutrition Support

What does this mean for Medicaid members?
• If you are a Medicaid member, your health plan is already working with ModifyHealth to connect you to services under this program.
• This program does not change your current health plan benefits or programs like SNAP, WIC, etc.
• It simply expands the services available to you.
• If you are already receiving certain services, you may not be able to get them again through ModifyHealth.
